COACH’S OBLIGATIONS • As a Client’s Coach, I will use my skills and knowledge to design a safe program that will take into account my Client’s lifestyle, personal goals, fitness levels, and medical history. • I will provide the coaching, supervision, advice, and support that a Client will need to achieve their goals. • A Client understands that the results of any health and fitness program cannot be guaranteed. • A Client’s progress depends on their effort and co-operation in and outside of the sessions. • In particular, a Client will acknowledge that individual results may vary and no particular result is guaranteed by their Coach. • All Client information will be kept strictly private and confidential.

CLIENT’S OBLIGATIONS • It is understood between the Client and the Coach that we both must commit to the Client’s training program 100% in order for the Client to achieve results. • A Client is required to arrive on time for each session so that complete coaching is achieved on each visit. • A Client is required to wear appropriate clothing and footwear. • A Client is required to complete a waiver and medical questionnaire before undertaking their first session. • The Coach may require a letter of ‘medical clearance’ from the Client’s doctor depending on the results of the medical questionnaire (please be aware that some doctors charge for providing this letter). • A Client understands and agrees that it is their responsibility to inform the Coach of any conditions or changes to their health, now and ongoing, which might affect their ability to exercise safely and with minimal risk of injury. • If the Coach requires further medical information from a practitioner, the Client must provide such details. • A Client understands that there are inherent risks in participating in a program of strenuous exercise. • If a Client sustains or claims to sustain an injury while participating in fitness sessions, the Client acknowledges that the Coach is not responsible, except where the injury was Glow Fitness Ltd. caused by the Coach’s gross negligence or intentional act. • The Coach cannot be held liable in any way for undeclared or unknown medical conditions. • The Client is responsible for keeping all their contact information and preferences up-to-date with the Coach.

CANCELLATIONS | REFUNDS | EXPIRY DATES • Personal coaching services are 100% non-refundable. No exceptions. • All coaching packages have an expiry date. Coaching packages expire within a certain range from the date they were purchased, and this range is advertised with each package. This is to ensure clients do not delay the utilization of their package. It is the Client’s responsibility to understand when their coaching package expires and book all their sessions immediately to secure their spots. Sessions that were not utilized before the package expires will be lost. No exceptions. All sessions expire 1 year from purchase date. • Booking appointments. Please contact Glow Fitness Ltd via text, email, phone or Facebook messenger to book appointments. There may also be time in person before or after sessions to book future appointments. • Clients should arrive on time to ensure they get the full session. The clock starts and ends at the time the session was booked. If a Client is late the session will not be extended. If the Coach is late additional time will be added to the session or to subsequent sessions. • 24 HOURS NOTICE IS REQUIRED TO CANCEL A SESSION. 24 hours notice is required to cancel a session with the Coach. It is the Client’s responsibility to ensure they rebook their canceled sessions. Clients who cancel an appointment within 24 hours of their session WILL LOSE THAT SESSION FROM THEIR PACKAGE. However, I do realize that there are life circumstances that do arise. Please contact me if you are sick, injured, etc so that we can discuss and decide a path forward. I am a mother of 2 young children and may have to cancel from time to time due to kids being sick. • If the Coach goes on vacation, gets sick, or has to temporarily stop coaching services for a period of time, all in-person Clients will receive a time extension on their package equal to the number of days the Coach is unavailable. • If a Client needs to take a break from coaching, such as vacation time, traveling, etc, the Client must let the Coach know in advance.

TRANSFERABLE A Client can transfer their unused portion of their coaching packages to another individual. It is up to the client to deal with money transfer with the other individual. The coach will not get involved with that.

TEXTS | EMAILS | PHONE CALLS • Texting, emailing and phone calls are reserved for booking, canceling, or rescheduling appointments. • If you have a question please send me a Facebook message or e-mail. I will respond as quickly as I can, however I do have a young family and we are very active. I will try as hard as I can to respond as quickly as I can.

SEMI-PRIVATE/ SHARED SESSIONS Sessions can be shared up to 2 people. Each client will be charged .5 of a session each time they train together. The coach will keep track of the sessions and keep the clients notified. If one of the clients calls in sick and cannot make it to the scheduled session, the other client has the right to: Cancel the session and not be charged Use 1 full session (2 x .5 sessions) and train with the coach by themselves one on one. If one of the clients cancels last minute or due to something that causes them to loose a session, the other client will not be charged for 1 full session, they will be charged their regular .5 of a session.

HEALTH & SAFETY The Coach has completed and holds a current certificate for Emergency First Aid & CPR. The Coach has liability insurance. The Coach provides a safe exercise environment. *****Please note that I am not a physiotherapist - I am a personal trainer that specializes in corrective movement.*****

LIABILITY DISCLAIMER & USER AGREEMENT Glow Fitness Ltd strongly recommends that all users, the “User”, of programs, or services first consult with their physician before beginning any exercise or nutrition program. By participating in any in-person program or service that Glow Fitness Ltd offers, the Client or User is declaring that they have definitely received permission from their physician to participate. A Client or User should be in good physical condition and be able to participate in any exercises or nutrition program. Glow Fitness Ltd is not a licensed medical care provider and represents that it has no expertise in diagnosing, examining, or treating medical conditions out of my scope of practice, or in determining the effect of any specific exercise or nutrition on a medical condition out of my scope of practice. A Client or User should understand that when participating in any exercise, exercise program, or nutrition program there is the possibility of physical injury or illness. If a Client or User engages in any exercise, or exercise program, or nutrition program they automatically agree that they do so at their own risk, are voluntarily participating in these activities, assume all risk of injury and illness, and agree to release and discharge Glow Fitness Ltd from any and all claims or causes of action, known or unknown, arising out of Glow Fitness Ltd negligence.
